Youth Sports Player Evaluation Template

A reusable player evaluation template for youth sports tryouts and practices — criteria, scoring scale, multi-station workflow, and parent reporting.

What a strong evaluation template includes

A youth sports player evaluation template should list skill criteria, a clear 1–10 (or similar) scale, space for position or role, and short coach notes. Soccer clubs often include technical skills plus soccer IQ; basketball and hockey programs adapt the same structure to sport-specific skills.

Templates fail when every coach invents their own spreadsheet. Shared criteria let volunteer evaluators score at stations while the head coach watches the whole session.

Run multi-station evaluations

Split the field or gym into stations so more kids are scored in less time. Assign an evaluator (or tablet) per station, rotate players on a timer, and stream progress back to a coach dashboard so nobody wonders who still needs scores.

PIN-login stations help when volunteers change mid-session. No app install required — just a browser and a short code.

Turn scores into development conversations

After tryouts or mid-season evaluations, share a parent-facing summary: strengths, focus areas, and next steps. Keep internal coach notes private. That builds trust for soccer, basketball, volleyball, lacrosse, and hockey families alike.

Re-use the same template across seasons so year-over-year player development is comparable — not a new scoring system every fall.
